As a Case Manager, you will play a vital role in providing essential support to patients facing various serious mental health challenges including homelessness.
Your responsibilities will include:
Meeting with patients on a one-on-one basis to offer compassionate support and guidance.
Facilitating group sessions to foster a supportive community environment and encourage peer interaction.
Offering resources and referrals to individuals struggling with mental health issues, empowering them to access the help they need.
Qualifications:
High school diploma required.
Previous experience in mental health support for adults or counseling
A minimum of 1 years experience as a case manager working with individuals with severe mental health illness or related field.
Strong interpersonal skills and ability to empathize with individuals experiencing mental health challenges.
Excellent communication skills, both verbal and written.
Ability to handle crisis situations with composure and professionalism.
This is an ONSITE role, Monday to Friday
