We are seeking a compassionate and dedicated Case Manager to support individuals experiencing homelessness in the Los Angeles area. This temporary role is ideal for professionals who are committed to making a meaningful impact by connecting vulnerable populations with critical resources and services.

Key Responsibilities:

  • Conduct client intakes, assessments, and individualized service planning

  • Provide ongoing case management and support to individuals experiencing homelessness

  • Connect clients to housing resources, healthcare, mental health services, and employment opportunities

  • Maintain accurate and timely documentation of client interactions and progress

  • Advocate for clients and collaborate with community partners and service providers

  • Monitor client progress and adjust service plans as needed

Minimum Qualifications:

  • At least 2 years of experience in case management, social services, or a related field (preferably working with homeless or underserved populations)

  • Demonstrated compassion, empathy, and commitment to serving vulnerable communities

  • Strong organizational, communication, and interpersonal skills

  • Ability to manage a caseload effectively in a fast-paced environment

  • Knowledge of local resources and service systems in the Los Angeles area is a plus

Preferred Qualifications:

  • Bachelor’s degree in Social Work, Psychology, Human Services, or a related field is a plus

  • Experience with HMIS or similar case management systems

  • Bilingual skills are a plus
