LHH seeks a Case Manager in Oakland, CA.
Responsibilities
Reach out to patients via phone and in-person at community sites who qualify for the program.
Help patients with referrals for resources and assist with applications and paperwork.
Assist patients in accessing public benefits (e.g., Medi-Cal, SSI/SSDI) and affordable housing.
Stay updated on patients' treatment plans and support their service utilization.
Join team meetings to coordinate care and reduce service barriers.
Advocate for patients and teach them advocacy skills.
Manage cases of patients with complex medical or behavioral health conditions.
Provide housing case management and help with document readiness and housing problem solving.
Offer crisis intervention and de-escalation.
Deliver and document billable services in required data systems.
Qualifications:
Passion for working with low-income individuals from diverse backgrounds.
Commitment to harm reduction and patient-centered care.
Ability to work independently with professional judgment and in a team environment.
Requirements:
High School diploma or GED, Bachelor’s Degree in Social Work, Health, or Human Services field.
A minimum of 2 years of experience in healthcare or social work settings, or 1 year as a Case Manager I.
Proficient in Microsoft Office applications.
Reliable transportation with a current license and insurance.
Preferences:
Experience with homelessness, mental health services, or addiction, either personally or through someone close.
Bilingual/Spanish is a plus.
Onsite, Monday to Friday. 8-5pm Oakland, CA
