We are partnering with a mission?driven nonprofit organization in San Jose to hire an experienced Case Manager to support adults experiencing homelessness. This is a meaningful opportunity for professionals passionate about housing stabilization, advocacy, and helping vulnerable community members rebuild their lives.
About the Role
As a Case Manager, you’ll work directly with unhoused individuals to assess needs, connect them with vital resources, and guide them toward stable housing and improved well?being. You’ll collaborate with community partners, crisis response teams, healthcare providers, and internal staff to deliver comprehensive, person?centered support.
Key Responsibilities
- Conduct intake assessments to identify client needs, barriers, and strengths.
- Develop individualized service or housing stability plans with measurable goals.
- Provide ongoing case management, advocacy, crisis intervention, and emotional support.
- Connect clients with housing programs, benefits, mental health services, transportation, and community resources.
- Maintain detailed and compliant documentation, progress notes, and updates in the agency’s case management system.
- Coordinate with shelters, outreach teams, and social service agencies to support successful housing outcomes.
- Monitor client progress and adjust service plans to ensure continued stability.
- Experience working with homeless populations, behavioral health, housing programs, or social services.
- Strong crisis intervention, de?escalation, and case management skills.
- Ability to build rapport with diverse individuals in challenging circumstances.
- Reliable, compassionate, organized, and able to work independently.
- Bilingual skills are a plus (not required).
- Contract role with opportunity for extension
- $26–$31 per hour, depending on experience
- Full?time hours
- On?site and/or community?based work in the San Jose area
